In March of 2002 The Joint Commission launched a new patient safety campaign called Speak Up^TM. Since that time, The Joint Commission has launched four more Speak Up^TM programs on wrong site surgery, organ donation, infection control and medication mistakes. The aim of these campaigns is to encourage patients to become an informed and active member of the health care team.

The following Speak Up^TM Initiatives are available for The Joint Commission accredited organizations.

Help Prevent Errors in Your Care: For Surgical Patients—Includes tips to help patients prepare for surgery and make sure that they have the correct procedure performed at the correct site on their body.
Four Things You Can Do To Prevent Infection—Highlights four easy things everyone can do to prevent infection and help avoid contagious diseases like the common cold, strep throat, and the flu.
Things You Can Do To Prevent Medication Mistakes—Presents questions to ask at the clinic, hospital, doctor’s office, or pharmacy to prevent medication mistakes; includes a wallet card to list medications.
What You Should Know About Research Studies—Provides guidance and a list of questions to ask to help prepare individuals considering participating in a clinical research study.
Planning Your Recovery—Includes tips that patients can use to find out about their condition, new medicines, and follow-up care so they can have the best possible recovery.
Information for Living Organ Donors—Provides basic facts about living organ donation, how to prepare to become a living organ donor, and questions to ask the doctor.
Help Prevent Medical Test Mistakes—Includes questions and answers to help patients prepare for important medical and laboratory tests so they can get the treatment they need as soon as possible.
